INSURANCE INFORMATION

The following are insurance companies that we can bill as a courtesy to you. Please check your plan carefully as you are ultimately responsible for payment of services received. We understand asking for a prescription is another thing on your "to do" list. However it is necessary so that we have the proper codes to bill. Please help us to continue to accept insurance by bringing a prescription even if not necessary, (you can usually just call your MD and they will fax, or e-mail the necessary form with codes to our office). Also  bring a copy of your card and any applicable co pay.
